WAIVER AND RELEASE OF LIABILITY
I acknowledge that COVID-19 is highly contagious and known to spread from person-to-person contact. I further acknowledge that by attending any event or service at the North Jersey Vineyard Church, or otherwise entering the church’s premises, I agree to abide by the procedures established by the church, and I voluntarily assume the risk that I and/or my family may be exposed to or infected by COVID-19 when on the church’s premises.
I also acknowledge that the risk of becoming exposed to, or infected by, COVID-19 may result from the actions, omissions, or negligence of myself and/or others, including, but not limited to, church employees, contractors, volunteers, members, and participants and their families.
