We’re so excited to hear of your desire to be baptized at the North Jersey Vineyard! This is an amazing event, taking this next step on your spiritual journey as you give your life to Jesus, and we are honored to be a part of it!

Please take a moment to fill out this brief form with some important information so we can make sure we are able to fully assist you. After you submit the form, please CLICK HERE to email Sue Sehulster a good quality photo of yourself so we can create a custom slide for you on Baptism Sunday.

A few things to note:

  • We ask that each individual fill out their own baptism form. If you would like to be baptized with someone else (i.e. a child or family member) or if you are submitting the form on behalf of someone else, please include that information on your form. One form per person to be baptized helps us serve you better.

  • The minimum age for a child to be baptized is 10.
